The Oregon Urology Alliance will be closing as of March 21, 2025.

If you have not already been referred to another urology practice and you desire transfer of your urology medical records you have two options:

• Your medical records will be available by emailing a Release of Information Form to [email protected]
• Another option to obtain your medical records is to mail a Release of Information Form to: 333 S. State St, Suite V PMB 336, Lake Oswego, OR 97034

It is very important to specify on the Release of Information Form the doctor's full name, the doctor's office address, and the doctor's fax number where you would like your medical records sent.

Please allow up to 30 days for receipt of your medical records to the office you selected.
